We always see the plumbers screwing up our installation so this made me chuckle.

It does prompt a few questions as an inspector.
Are those 2 LBs accessible?
I assume the top screws are missing 110.3.B

What is in that pipe and will that flange leak?
Is this a wet location?
Is that a fire rated wall?

Brilliant! I bet he used a few discs on the disc grinder to cut through that solid gland.

The middle LB top screw on the cover looks like 'no way in hell' to get at.

Now, who was there first?? My money would be the LBs and conduits were there first, and the pipe fitters did the 'notch' in the flanges before the pipe was put up.

Look carefully at the notch and how close the flange is to the wall to be able to do the cut.

I guess that is why they call them pipe fitters,. They can make it fit.
