I guess I need to study more about this. Jan. 2008 NEC, dwelling unit application. How can AFCI/GFCI protection be provide for (example) kitchen counter receptacle. Are there going to be combination AFCI /GFCI circuit breakers available to purchase?

That is the easy part.
AFCI breaker and a GFI receptacle.
Now make the receptacle tamper proof.
Now explain to the customer why this is needed.

If it was easy, anyone could do it.
