My boss had found this service installed back in 2004 by an electrian in Dudley, MA. This was approved by the AHJ although there were a few things we felt are not correct.

i dont know if possiblty the fuel oil vent is too close to electrical vents but i dont know what the inspectors will say about fuel oil vents near electrical meter[s] most of the time i usesally see are pretty good distance away from electric meter just like natural gaz rules are useally keep min of 3 feet[1 meter ] away from meter or anysource that make spark. the other thing is the fuel oil vent pipe is too close to basement window and first floor window if the fuel oil get too full and spill over and whooo you get a wiff the smell of fuel oil or diesel fuel one of the two [Linked Image]

SE cable appears comming out of the disconnects.
Code requires SER cable providing seperate ground after the disconnects.
Also within 3 ft. of area susceptible to physical damage.
