This is a 3 phase ML panel that had the 1" EMT shoved up through the bottom by a fork truck. There were originally two 4" pipe bumpers bolted to the floor 4' in front of this panel. They were removed so a skid could be set in front of the panel for scrap wood. When the driver went to pick up the skid Monday AM his forks were sticking through farther than he thought. He was under the 90ยบ sweep of the 1" EMT and just drove it up into the panel when he picked up the skid. This is a good example of what can happen when workspace clearances aren't maintained. 110.26
This photo demonstrates (again) that while you can make things accident & idiot proof, you cannot make anything proof against deliberate malice. There were protective barriers? They were removed? To make room for scrap? I thought that "management" personnel were paid to exercise their judgement, and supervisors to supervise? Yet, if anyone was fired, I'll bet it was the poor fork operator- who would have shown a "bad attitude" had he objected! Fine. Red-tag. Sue. Charge double.
When I was working inside as a maintenance super., Ther was nothing safe from hilo drivers! Floor mounted transformers, panels sprinkler pipes,or doorways. Any protection placed in front of an object was fair game. They broke 4" filled pipes, moved panel boards, transformers, desks and fell off the dock loading moving truck!
