Check section 300-15(a)1999 NEC where it says that "Where the wirinng method is... nonmetallic sheathed cable, or other cables, a box or conduit body complying with article 370 shall be installed at each conductor splice point..."
Awhile back, the cubic inch space in the luminaire canopy was excluded, when doing wire count / box capacity checks. As a result, even the 4" pancake no longer has the volume needed to run a 14 NM into. Clamp + ground + hot + neutral = 8 cubes.

Sorry if I sounded argumentative. I didn't mean to. I just wanted to be thorough and was musing whether a box recessed in the wall is actually outside the house.
