I believe NEMA once published a guide as one of their "AB" standards, but can’t seem to locate it at their site.

Does anyone know of a way to check for a line to line fault on a 240 circuit besides disconnecting the load and checking for continuity between the two legs?

“…way to check for a line to line fault…” Insulation-resistance tests applying less than 500V should not be considered safe or reliable. ø-ø and ø-g checks are important and are considered routine by many.
