As John said, each POCO has their own rules and you have to follow them, if you don't you have to wind up removing the ofending piece and doing the job all over again.
I install an underground service once and it met all of the NEC requirements, however it didn't meet the POCO rules and I wound up digging up the underground line in the middle of the winter to redo the job because the POCO didn't like it.
