I've never worked with two phase myself, but have heard from others that have. Always in areas of PA. For one thing, they use 2 phase 5 wire for the most part, which is two sets of hots and a grounded neutral that is common to both sets. Apparently, the only time the "two phase" with all four hots is actually used is motors. Otherwise either set is used to power normal single phase panels for lights and receptacles. I guess they can still order 4 pole disconnects. There are even some local manufacturers in that area that custom make panels if needed.
