In Minnesota, the FHA has a standard form, and a set of minimum electrical standards. The form is called, simply, "Electrical Certification". Sometimes the minimum electrical standards are printed up on the back.
In the past, I've gotten the most current from the realtor involved, or, sometimes, the local authority (building department) has their own requirements and form that supercedes the FHA Electrical Certification.
HUD lists three home offices in your region. I've emailed a copy of that info,
from their web site . Looks like you are in region V.
