Why did it blow up ?
Possibilities which could have caused it.
1. Sustained continuous overload, and not monitoring oil temperatures and winding temperatures.
2. Buch holz alarms, faulty which is rare but could be because of lack of maintenance.
3. No oil samples taken over a longer period of time. ( In NZ it is practise to take oil samples twice a year ).
4. Oil pump not working, should raise an alarm to network control.
5. Forced air ventilators not working.
It is interesting that these events just happen. If equipment is monitored and maintained a transformer will give early warnings that there are problems ahead.
