Is that a 3-prong plug on 2-conductor zipcord? That makes about as much sense as the existance of plug adapters.
...actually, I'm trying hard to think of any actual codes this install broke! A (I assume) listed lamp base adapter was screwed in, and the plug adapter appears to be used correctly. And the only thing plugged in was a light. It might be skirting a requirement for a dedicated receptacle, and certainly not professional-looking, but is the installation actually unsafe?
