It looks like the cover on the "LR" conduit body couldn't be removed.
Unused openings in box aren't sealed.
Sorry, Gus, but I must disagree with you. All the conductors appear to be THWN, 600V rated, and may occupy the same raceways or enclosures (see 300.3(C)(1))...S
