usually I have seen the L1, L2, 3, 4 wired as :
for 120- L1&3 together, and L2&4 together
for 240- L1, 3&4 together, L2
but the other wires throw me off- is this an OEM type of application? Is it indeed a reversible? Is it a PSC motor? What is the application?, Can you back into it through a mfrs. schematic? Unless it has a odd frame # or special shaft, this motor shouldn't cost too much to replace with new one, rather than spend too long on diags.
