I saw an interesting product. Bridgeport just got the Mighty Rite split bushings listed. They sell a wrap around insulating bushing and a grounding bushing that can be installed after the wire is pulled. It might be handy for one of those "oops" moments.

SqD has also released their combo GFCI/AFCI breaker for NEC 2014. It comes in QO and HOM.

The grounding bushings first came on the market in late 2012, as far as I can tell. A you tube video advertising them. http://www.youtube.com/watch?v=_V33Ey9bMT4Did the insulating bushing version just get recently listed?

Those figure to be great when cleaning up the Other Guy's work.
In new work, plain vanilla bonding bushings are always installed before the conductors are pulled.

As an inspector it would make me feel a lot easier about calling the violation that has existed for a decade. It is only a little money, not pulling a 750MCM feeder out.
