For utility-type 2.4-34.5kV distribution, it is discussed in IEEE Standard C62.92.4, which is outlined at http://standards.ieee.org/reading/ieee/std_public/description/surge/C62.92.4-1991_desc.html

Like most codes and standards, the text is not free—you must buy or subscribe to it, or find it at an electrical engineering library.
