Frank,
A PBX is a Private Branch Exchange.
It is used inside commercial buildings to distribute phone calls, with each phone being an extension of the main phone, I think. [Linked Image]

what exactly caused this fire?? as phones run on low powers and voltages, what happened? is there further explanation of the "miswiring"?

Nasty indeed. Shorting or crossing the actual phone lines wouldn't cause this.

The miswiring was most probably either:

(a) 120 or 240V AC power connected to the wrong points, or

(b) a 50V battery supply incorrectly wired and no proper fusing in place.
