OK Frank,
we have CT's, i see 4 wires, one possibly spliced thru. i see 208V colors, i think i see something green from the upper right of the box comming down.

I'm gonna go out on a limb here..

Are we dealing with a seperatly derived system, and violating 250-92?

Shame on everyone involved. The installer failed to bond the cabinet & the conduit nipples (they might be bonded at the other end, though I tend to doubt it). If this passed inspection, the inspector was asleep & if it was energized by the power company, they too dropped the ball.

250-92 is clear about what is to be bonded & 250-94 tells you what you can/cannot use. Plain jane locknuts certainly don't cut the mustard & the #2 jumper from the grounded conductor to the CT cabinet is also missing. Also, the wireway in the upper right corner must also be bonded (bet it isn't).

This installation has not been inspected yet. I took photo so I could present it to our City's Electrical Inspection Department. It's my job to review Contractor's work performed for our Department. I work for the City of Pittsburgh. The Contractor doesn't get paid if it is not a legal installation. It is clearly in violation of the section you mentioned Tom. It is a 120/208 Volt, Three Phase, Four Wire Service Lateral. The existing C.T. Cabinet was rusted. The Contractor installed a new one and reworked the raceway.

Is that a "Corner Grounded Delta Configuration" (tongue in cheek)?

Looks like a green tap on the A leg...

BTW...My wife is who spotted it!

I realize that these are supplied assembled, but is green the proper color for the CT tap?

Your wife has a good eye for noticing things. The green wire you see is a #8 AWG. One end attaches to the wireway in the upper right corner of photo. The other end attaches to a ground rod located beneath C.T. cabinet. I suppose that was an attempt to ground system.
