I find many of these bad installations during my inpections!

Some locknuts are installed backwards!

What's the problem? This violates a very simple rule .... continuity!

Guess it all goes back to that old saying "pride in workmanship". It seems to becoming a more "hurry-up" world. How about "quality instead of quantity" within reason?
Are/were the lnuts tight on the inside???

I did not remove the covers to check, but from the looks of it I would say that they were loose. I'll also bet that the required bushings according to 300.4(F) were missing.

is that rust on the nipple???? when was it installed???

what about bonding lock nuts

The rust on the nipple has probably been there about 10 years, and bonding locknuts will not help to correct the problem here.

The locknuts are loose and were never tightened. [Linked Image]
