200 amp service, in a strip mall,200 amp disconnect at meter base. service panel about 100 ft. away is there a code requiring a main breaker in the service panel? and if not would you do ?

There's not a requirement for an additional disconnecting means at the subpanel.
As long as the main breaker at the meter base is accessible to the occupant per 240.24(B), and 230.72(C) I would just use a main-lug-only subpanel.
If it's one of those places that (in violation) keeps an electrical room locked from the occupants, I'd probably add the main.

How would supplying a main breaker in the panel address the requirement in 225.35 for occupant access to the supply disconnecting means?

225.35 Access to Occupants.
In a multiple-occupancy building, each occupant shall have access to the occupant?s supply disconnecting means.
