I've got a old fan that is 133 cycles and wanted it to put a plug of this style to protect it from being plugged into today 60 cycles. Plus maybe make it look like it might have back in the day it was new.

Isn't that an Edison base (lamp) socket size?
I am not really sure where you find one but I would start googling "antique electrical" and see what you find.
There are some restoration places that might even have that cord. I haven't really done much personally but when they were renovating the Ringling Ca D Zan mansion in Sarasota there was a guy who turned up some pretty cool stuff.

If you ever have the chance, read "The Twilight of Splendor" by James T. Maher ISBN 0-316-54385-3 (It is out of print though) The 2nd chapter is on CA'D'ZAN & its owners, a couple of other chapters are on Edward Stotesbury who made & spent nearly a 100 million $$ in his lifetime & Hubert Parsons whose wife literally broke him financially when building his home in NJ, & driving the architect nuts w/ her crazy demands & ideas. (Those were the 3 I found most interesting.)

In the house I grew up in, the laundry area had an 'Edison base' outlet for the ironing board.

You could always get a 1-15 plug and glue it into a pignose. Alternatively, use a crowsfoot plug; I remember back in elementary school they used crowsfoot for 110DC and 1-15 for 110AC...

LED recessed can retrofits often have a cable with a screw-in plug on one end; the ones I have seen, from Sylvania, come with such a cable with the pignose on one end and Anderson Powerpoles on the other.
