This is often seen where a cable has been cut and must be joined back together or someones' idea of Tapping into a line.
Why is this Bad?

Back wrapped ECG's and short conductors, exposed hots behind wire nuts. Could have been done with 2 junction boxes spread apart with 2 new pieces of NM to give adequate wire for the 3-6 rule.

Violations not withstanding, which Joule-e has already mentioned, this thing must have been VERY difficult to put together! [Linked Image]

assuming 1.5"x4" Octagon box and 14 AWG.
15.5 cubic inches / 2 Cu.In. = 7 14 AWG.

314.16.

4 - 2/C NM = 4x2 = 8 x 2Cu.In. = 16 Cu.In.

4 EGC's = 1 EGC = 2 Cu In.

4 Connectors = 1 connector = 2 Cu.In.

Total Cu.In. = 20 Cu. In..

Box is 4.5 C.In too small.

Plus 250.148(A).

Also looks as though some bare Ungrounded conductor exposed at the wirenut, possible 110.7.

Question 334.24.

Appears box was secured to the wood or maybe just a shadow over the bottom hole of the back of box.
