How would you write the violation notice?

It looks like this pathetic attempt at wiring has been in use for quite some time. The thing that comes to mind is whether this installation was ever inspected. I don't know of a licensed electrician who would do such shoddy work, nor, do I know of an inspector who would pass it. That is one ugly mess.

I would write this Violation on about 5 pages with a special number on all of the pages of the owner's copy... 911
