December 19th      Construction        Ottawa                   The lock on an electrical panel was cut off by a contractor.

                                                                                                whether the lock was a personal protection lock or an

                                                                                                equipment status lock.  If it was judged important enough

                                                                                                to report to the Ministry though, I suspect that it wasn’t

just an equipment status lock.  In February of this year,

there was another report of a lock and tag being removed.

 

 

Fatalities:

 

October 17th           Industrial               Napanee                                Worker was replacing defective relay switches for 347 V

Entry would was on left hand, exit wound was on scalp.

 

 

Critical Injuries:

 

December 15th       Construction         Mississauga         Worker working on electrical box, box blew up.  Worker

December 16th       Industrial               Kirkland Lake        Worker was working next to a powered bed.  One of the bed’s

                                                                                                arm and out his hip.  Worker sent to hospital, has problems

Nothing this quarter was sufficiently off-the-wall to be included here, but the H&S Manager who lost the tip of his finger while doing a demonstration of safe work practices around a stamping press did came close.
