December 19th Construction Ottawa The
lock on an electrical panel was cut off by a contractor.

whether
the lock was a personal protection lock or an
equipment
status lock. If it was judged important
enough
to
report to the Ministry though, I suspect that it wasn’t
just an equipment status lock. In February of this year,
there was another report of a lock and tag
being removed.

was replacing defective relay switches for 347 V

Entry would was on left hand, exit wound was on scalp.

December 15th Construction Mississauga Worker working on electrical box, box blew up. Worker

December 16th Industrial Kirkland Lake Worker was working next to a powered
bed. One of the bed’s

pierced an electrical cable, energizing the frame. The
worker
contacted the frame, current went through the worker’s
arm
and out his hip. Worker sent to
hospital, has problems
with
co-ordination.

Manager who lost the tip of his finger while doing a demonstration of safe work
