This article governs the use of devices "of insulating material" and that it shall be "permitted to be used without boxes in exposed cable wiring and for rewiring in existing buildings where the cable is concealed and fished."

It sounds like they're talking about surface mount devices designed for use with NM cable?

The NYC ammendment says that use of these devices are not permitted. That would explain why I've never seen these things.
