We had that picture once as part of a series if I remember correctly... the place burnt down, probably because of this "fuse replacement". These fuse holders are built like Edison base light sockets, but they have a porcelaine ring at the bottom of the element, so only a proper fuse with a narrow tip will fit (the diameter depends on the fuse rating). The bottom terminal is the line side, the outer thread is the load side. I can only assume someone wedged a metal object into the base and used the screw driver to both "secure it" in place and make the connection with the screw thread.
