What, in your opinion, is the most overlooked code?
IMHO, the one mostly ignored here is strapping of conduits by electricians. When it comes to John Q Public, it seems to be workspace.
Close race between these here in WV:
1) Box fill
2) Conduit fill (NM Cable especially)
3) Derating of bundled conductors
4) Proper cover for buried pipe or cable

This is impossible to narrow down to one or two, but these make my list
Lack of wirenuts on equipment ground splices
Equipment grounds not connected to boxes
Multiple wires on/in a terminal
Box fill
Working space/clearance
Working space/clearance (keep repeating this about 100 times)
Oh, so that's what the green screw is for (as applied to panelboards)
Grounded conductors used for grounding conductors.
