I wonder whether the switch to which this was attached was already marked "on" and "off" on the casing?

More than likely.

My guess is that the installer of the heater tagged the switch off, with the customer to turn it on.

Childhood memories!

PECO customers in the 1960 through at least the 1980s had a dual meter setup as they offered off demand rates for electric water heaters. The second meter fed a tamperproof glass enclosed timer hanging from the meter box. This in turn fed a 30a fused disconnect for the water heater. I assue the label would be placed on the disconnect.

Hey Gus, every mobile home in this park with oil heat still has two working meters on the pedestal, one with a working timer attached. I live in one of 4 1/2 sections to have gas heat. I'll get a pic of one in an empty lot some time this week.
