Of course I didn't have my camera with me, but during a service call to a house, I found an L/B nippled into the meter pan, the pipe went into the attic to a juntion box. It was a 20-40, 200amp murry panel, with about 16 spaces left, as long as you got twin breakers. But someone added a sub-panel next to it and tapped off the line side.(using #6 thhn doubling under the feeder lugs of the main breaker.

...just another example of some of the stupid things people do to avoid paying an electrician and having the job done the right way. I thought you were going to say that the whole sub-panel was tied into the line side at the meter pan. I've seen that before!
