Wow. I had ordered some electrical parts in the mail a few days ago and received them yesterday.

One of the parts I needed was an "RV plug" (the funky two slanted pin + 1 round pin 30-AMP/125 volt jobs). This is what they sent me instead, by mistake:

This thing is HUUUGE. Three-phase 20 amps at 120/208 volts!!

What would it have been used for, originally?

I just saw one of these for the first time last week, it was from an APC UPS that had cord and plug connections to each of the server racks. The strip in the rack was three phase with 1/3 of the outlets on their own phase.

Looks like a NEMA L21-20.

Used in "pendant" applications in machine shops quite a bit, in addition to power cords on large computer racks.

Pretty much the standard connector for cord and plug connected 3 phase equipment that requires a neutral.

A lot of shops use this connector in place of a motor disconnect for 3-phase fractional horsepower devices

That's not huge. I have an IEC309 120/208 @100a laying around in my garage. THAT is huge.

we use a plug similar to that on the end of the shore cords at the boat manufacturer i work for. except our cords are single phase

Sven:
Long time, no type! Hey, don't shop via the mail, go to a local supply house!
(Just kidding)

Usually, 120/208 3 phase, 4 wire, with a GROUND! 3 'hots'; 1 neutral, 1 ground

As everyone else said, it ain't that big, and it's not that unusual. (It's NEMA)
200 amp, 3 phase, w/grd., 480 volt Russel Stohl....now that's big & $$$$$$$ (MRI trailer unit hook-up)
