Attached are photos from 2 jobs.

The lamppost and grounding are from a home that is full of this type of horrors. I have been instructed by the new homeowner to make all necessary repairs.

The service is an old apartment complex that I am upgrading, I will submit more photos and comments soon.

I am almost ready for inspection on the service and yes those are solid limestone walls. 24" thick to be exact. We drilled 5 2" holes to run the pipe thru, what a day that was.

No doubt the combo device was grounded to the neutral to pass an inspection using a receptacle tester. I've seen it done before; the first time was about 30 years ago, when I was a helper, by my own boss!
