I,m a journeyman in a large industrial facility and am encountering floor mtd rails mounted so close to 480 panels you can't comfortably open the access door. These have removable cross rails but wouldn't this violate "readily accessible"

If it is an installation that you are in charge of, do it to code. If it is a pre-existing condition that you did not install, see-no-evil...
