Over here in NZ, the Energy Safety Service is responsible for safety pertaining to Electrical and Gas Installations.
They occassionally release a document called the Summary of Electrical Accidents.
It can be downloaded at www.ess.govt.nz/safety/pdf/summary_of_accidents_2001.pdf
This document is a PDF file, but it is well worth a read,I feel, as it shows (sometimes silly) mistakes made by people that perhaps should know better, over here. [Linked Image]
There are also gas accidents in this document as well, some of which would make your hair stand on end.

One aspect seems consistent with North American concerns.

For roughly 100 years in low-voltage [<1kV] situations, the primary electrical hazard was almost universally considered electric shock. In most regions over the recent decades, arc-flash problems are increasingly getting their due in occupational training and accident investigation.
