JBD A single throw inverted design where the fuses and the switch blades are "above" the line contacts. Bolted pressure type switch blades are allowed to be barriered. Have you ever seen any of these in the field? I have installed and serviced a large number of disconnects and have never seen one designer for bottom feed.

I think 404-6(C) discusses this, not line and load per say, but that the breaker terminals to the load must be de energized with the breaker in the open position. That is probably why the line in terminated where it is, by design of the breaker.

Trekkie, in a feed through breaker the terminals to the load would be deenergized if the other end terminals were fed from the source, this would be regardless of which end terminals were used. Here , here , here , and here are some answers to this from SQ D Roger
