Question: Does a outdoor hot tub need a disconnect as per 680.12? and does it need a gen purpose receptacle as per 680.22(3)? This is to be installed at a single family residence outside on a deck. Also this is a manufacture home if that matters. Thanks

Yes and yes. see 680.40 and 680.42 And it doesn't matter that it is a manufactured home.
