I see a couple of issues in the larger images:

White wire on top-right breaker w/o being re-colored.

It looks like that breaker is a Thomas & Betts.

What about the re-used breaker on the left?

Back-fed breaker as nain w/o hold-down hardware.

Two aluminum conductors in one top-right terminal. (Anti-ox?)

Actually they look like forgeries. If so the labeling is also forged and probably have never been tested for anything.

Is that the transformer for the doorbell that is hooked directly onto the box?

Yes, but that is not a violation.

Heh. Didn't think it was being that the panel is a j-box but hehe. Man if I did that I would never hear the end of it.

Hmmm, looks like the black from the doorbell transformer is together with the branch circuit on that last breaker. Two wires on the same lug.

Look real close. You can barely see it. Reference the other c/b's in the larger photo. Maybe the lighting makes it hard to see.

They looked good to me at first until I looked up 240.81

Sandy, if you look carefully, it appears someone (Joe?) has covered the words "on" and "off" with little rectangles of electrical tape.

I know for a fact that the Siemens and Murray breakers that use these cases have the words molded into the plastic; look next to the CSA.
