Here is a couple I heard of:

commercial work: Maintenance guy goes to mall pdc to check on a problem. Went to remove cover from panel and when he did there was a screwdriver that had been left behind on a ledge, the screwdriver rolled off and got across the phases in the panel. Blew out throught the front and into his chest. Did not die but had hard time getting compensation...turns out they ended up suing sq-d for failing to provide a barrier and won a settlement..still crippled up..

nuclear plant outage: journeyman and helper mounting unistrut. helper drilled into the backside of a 13.8 cabinet and into the feeder, the j-man was around the corner and heard the explosion and the flames, he ran around the corner and saw the apprentice on the floor...the j-man ran and grabbed the cord of the drill to unplug it, he too was electrocuted and fell on top of the apprentice...both died

papermill in maine: there was a freight elevator that was used by construction hands to carry up materials to the top of a boiler and a digester..when the door of the elevator was closed there was no screen in the window..it was a common practice to lean out the window to see where the next floor was or whatever...a new apprentice saw his j-man do this for several days...one day he decided to try it...he stuck his head out a little too far and just below the next floor and got his head caught..dead on the spot..


pennsyalania power house: flatbed of conduit rolls in...foreman send a couple green apprentices over to help unload..they went around one side of the truck and decided to undo the binders, as soon as the first binder was undone the conduit all rolled off on top of them..dead on the spot.

foundry: a long time friend and well respected electrician was found dead laying under a transformer after he had went out on a late night call. somehow he got into the feed and got nailed...

one close call I had was in wyoming at a gas plant, where a green hand had just gotten a new fluke meter, he was running around bragging about it and checking everything he could, later that day we were doing fco and he was helping someone check to see if the circuit was dead..480 vac...so he ran up to the mcc and stck his meter across A phase and another A phase and shouted...IT IS DEAD!...the guy he was helping was smart enough to NOT TRUST ANYONE decided to check it for himself to make sure...sure enough...it was energized..

the kid learned a valuable lesson there...

another one was where a bucket in an mcc was mislabed...the hand thought he shut off that circuit but since it was mislabeled he had shut down another bucket...

you really gotta be careful!
