Stop. Receptacle boxes in the floor gotta be approved for use as floor boxes. Check article 314.27(C). You cannot use regular wall boxes.

I think I'll do something like e57's picture shows (thanks e57). I'll just strap a pvc 90 to the concrete form where it will end up in the "Sleeper Recess" from there I will take the earlier suggestions, and pipe over to where i'm going to mount the boxes, and fasten some type of floor box directly to the sleeper.
I'll have to do some research on what kind of floor box will be best for this situation.

Not just the box to be rate, but plate too.
It must be able to withstand a 400 lb person standing on one high heel shoe!
Or something to that effect. No there are specific box and plate ratings out there.... Some for in use too!

Just saw it at the supply house this morning.... a floor recep from P&S (pass & seymour) Brass plate, single recep & STANDARD 3 1/2 " sw box included. FWIW.
Shoot first, apologize later.....maybe

P&S makes a floor receptacle that will fit on a standard box. Use one of the adjustable single gang side mount boxes. or a new smart box would work after the fact.
