Back when I worked at one of the Ford plants, every one of the Tradesmen had an AVO book in their back pocket (Avoid Verbal Orders).

If someone came up with a task for them to do, they were instructed not to proceed until someone put in in writing and signed it.

...Amazing how many dumb ideas went away when you asked for a signature...
