This is a pretty obvious violation. We found it (among others) surveying the service entrance equipment for a train station in New York. This disconnect switch (see photos) is the main service disconnect for a small concession stand at the station. The disconnect is located in the storage room for the concession stand.

This gives me flashbacks of one of the "big buy membership" stores where I went to do a troubleshoot once... They turned the electrical cage into a storage for high theft/high dollar items... Big steel racks "bolted" to the floor in front of 3 panels! [Linked Image]
