This looks to be as mucked up as a single fitting could possibly get!.
This must have caused some real confusion to the person that opened it up first.
Personally, I wish we'd stayed with the Green ECC, having a green stripe on a yellow wire can cause confusion, if you can only see the wire from one angle, or there is the problem of having to put your hand in the enclosure to turn the wire to see if there is a green stripe on it.
One question, is that some sort of conduit that the load side cables are leaving the enclosure in?.
They look like SWA cable glands.

Ditto! I wish we'd stuck with the old system for everything. A lot of earths (or sleeving for earths) now seem to be predominently yellow with a much smaller green stripe. As you say, if it's turned the wrong way it can look like plain yellow at first. What was wrong with plain green?

Looks like SWA glands to me, with T&E just threaded through them. I found the original thread in which this appeared, after a little searching:
