A little detail here....the pic of the cover is a cover for the larger "4-0" box. The duplex receptacle will not fit in a 3-0 unless installed corner-to-corner. So far, I have only been able to find covers for a 3-0 that have a single receptacle mounted on them.

The cover I removed was for a 3-0 box, the recept mounted to the box as in your photo and the cover had provision for the 6/32 in the recept only. I should not have thrown it out for the sake of this thread.

Joe : i am well aware with this type of cover and iam not crazy with at all{ dang i am swearing myself how to tranfser the photo here so ican show you more clear idea with it ] i know the Raco company did have diffrent cover for 4 inch octganal box it have 3 screw holes for it , 2 for yoke and center one to hold the repecticale but for some reason i did look at the catalog book it only show the same as Joe's photo have it but i am sure there is other company do make it with 3 holes there
