iwire--since you brought up 406.8(C) in another thread, I read (D) and am slightly unsure what 'standpipes of floor receptacles..' means.

I'll take a guess it is a older code rule for some sort of receptacle box supported on a pipe nipple out of a threaded floor system like walker duct.

Larry and Earl, we found these in a university remodel. At first I wasn't sure either, but we found that they were mounted to floor duct through a nipple and the 1" RMC coupling that you see in the picture.
You can see the two screws that hold the top canopy on the fitting.
When the top is removed the devices can be wired (by the way this is a duplex receptacle, there is one on the other side too) and then the wires pulled back as the devices are fit back into their place in the fitting.

If that type of install is what is meant, then my question is-- How high off the floor does a receptacle have to be mounted before it is not a floor receptacle?

IMHO If it's not in the floor then it's not a floor receptacle. If I'm the inspector, I'm just like the umpire- is it a ball or a strike, is it a floor receptacle or is not a floor receptacle.

As to floor recept...I doubt it.
