This photo demonstrates (again) that while you can make things accident & idiot proof, you cannot make anything proof against deliberate malice.
There were protective barriers? They were removed? To make room for scrap? I thought that "management" personnel were paid to exercise their judgement, and supervisors to supervise? Yet, if anyone was fired, I'll bet it was the poor fork operator- who would have shown a "bad attitude" had he objected!
